Diagnosis: Seeded choroid plexus papilloma.

Most choroid plexus papillomas are seen in children and tend to be in the trigone of the lateral ventricles in that age group. In adults, choroid plexus papilloma may present as a fourth ventricular or cerebellopontine angle mass. They may produce ventricular dilatation due to obstruction, or CSF production and may seed via CSF spread. Calcification may be present. Enhancement of choroid plexus papillomas tends to be a bit more homogeneous versus the heterogeneous enhancement typically seen in ependymomas.

The multiplicity and extraaxial nature of these lesions suggests a primary neoplasm which seeded via the CSF. Ependymoma, choroid plexus papilloma, medulloblastoma, and certain pineal neoplasms all tend to spread via seeding of the CSF. Note that the pineal region is ok. This patient had a choroid plexus papilloma removed from the posterior fossa ten years earlier. Related Cases
