Diagnosis: Cervical schwannoma

Schwannomas are the most common intradural, extramedullary neoplasm of the spine. They may also have an extradural component. Intense homogeneous enhancement with a dumbell appearance is typical. Hemorrhage and calcification are rarely seen in schwannomas however, 40% of spinal schwannomas may contain a cystic component. Schwannomas usually originate from the dorsal sensory roots and present with pain and radiculopathy. Meningiomas are the second most common intradural spinal neoplasm, and are more commonly seen in the thoracic spine. Enhancement in meningiomas may not be as striking. Also, meningiomas tend to have lower T2 signal versus schwannomas and usually have a broad, dural base without a dumbbell appearance. Metastasis much less likely, but may be considered if there is a history of a primary neoplasm. Related Cases
