Diagnosis: Lipomyelomeningocele with tethered cord
Lipomyelomeningoceles make up over 80% of spinal lipomas. They often present at a young age with neurogenic bladder. The filum terminale is usually tethered to the lipoma which is extradural, accounting for the low lying conus. Lipomyelomeningocele is associated with Chiari I (not II) and has syringohydromyelia in 25%. The remaining 20% of spinal lipomas consist of the filum terminale fibrolipoma, which is often incidental, and intradural lipomas. Related Cases
